Signals don’t all come from the same place, and knowing the full range of sources matters because each has different strengths, different biases, and different timeliness. Spontaneous reports — from patients, healthcare professionals, and regulators, flowing into company and national databases — remain the traditional core of signal detection, precisely because they capture real-world use across the broadest possible population, including patients who’d never be included in a clinical trial’s inclusion criteria.
Published scientific and medical literature is a second major source — individual case reports in medical journals, epidemiological studies, and pharmacological research can surface associations that spontaneous reporting alone might miss, particularly for effects that develop slowly or that clinicians wouldn’t immediately think to report as drug-related. Literature monitoring, covered in more operational detail elsewhere in this course’s broader curriculum, is a formal, systematic PV function specifically because of this.
Clinical trial data contributes in two ways: a sponsor’s own ongoing trials can surface an emerging pattern before the drug is even approved, and published results from other trials — sometimes trials of related drugs in the same class — can suggest a class-wide signal worth investigating for a specific product. And increasingly, real-world data — electronic health records, insurance claims databases, and other structured healthcare data — has become a genuine fourth pillar, offering scale and structured comparison that spontaneous reporting alone can’t provide, at the cost of the causality ambiguity that comes with observational rather than reported data.
A mature signal-detection function doesn’t rely on just one of these sources — it deliberately triangulates across them, because a candidate association that shows up in spontaneous reports, gets independent support from a published case series, and is at least directionally consistent with real-world data patterns is a far stronger candidate for validation than the same statistical flag arising from spontaneous reports alone.
