ICSR received from a spontaneous report, literature, or clinical trial and logged with a unique case ID.
Stage 2 of 8
Case is checked against the four minimum criteria: identifiable patient, reporter, suspect drug, and adverse event.
Stage 3 of 8
Adverse events and medical history are coded to MedDRA Preferred Terms for standardized analysis.
Stage 4 of 8
Relationship between drug and event is assessed (e.g. WHO-UMC scale) to classify seriousness and expectedness.
Stage 5 of 8
The clinical story is written in prose — onset, treatment, outcome — the human-readable core of the case.
Stage 6 of 8
Second-level QC checks narrative accuracy, coding, and seriousness classification before submission.
Stage 7 of 8
Case is submitted to authorities (e.g. FAERS/EMA) within required timelines — 15 days for serious, 90 for non-serious.
Stage 8 of 8
Missing information is requested and incorporated as it arrives — an outcome update can reopen causality or seriousness, feeding the case back into the cycle rather than closing it for good.
